Workbooks cost $12.00 each, with $5.00 flat rate US shipping. International shipping is additional.

Parish, Address, Location @ the Parish, Date, Time, Presenter Name, Registration Cost, Contact information for registration (phone number/email)
Please indicate in the subject line of your email it is a poster request.
Certificates
In order to receive certificates for your study, all you need to do is make sure you send us the registration cards which are included in each participant workbook. We will create the certificates for everyone and mail them back to you.
